Does my lab form expire?
Yes, lab forms at Quest Diagnostics expire 6 months after you "Print". If you believe your lab form has expired, you can check in your "My Products" tab, located on the top menu bar. If your lab form has expired please contact customer support here. We can send you a new lab form for your blood draw.
